Sridevi with her sister Sreelatha and mother Rajeshwari, father Ayyappan (that's how Sridevi gets her name A Sridevi).There's also Sridevi's step-brother Sateesh/Satish... who we know very little about. But he has been addressed in several interviews in the past by Sri e.g. here and here.
Sridevi's name would be "Sreedevi Ayyappan" on her passport, but in South India, the surname comes first.
